Families must first contact the Delaware County Administrative Entity Intake Specialist to determine if the individual meets eligibility requirements. Contact 610-713-2330 for information.

Once it is determined that you are eligible for supports in the Intellectual Disability system, you will select a Support Coordination Organization, meet with a Supports Coordinator who will develop a support plan, and will then coordinate supports for you and your family.

Services

The Supports Coordination Organization helps individuals and families coordinate assessments, connect with providers, and ensure effective services. Paid service options include community participation services, residential supports (LifeSharing, Family Living), in-home and community supports, companion services, respite, assistive technology, behavioral support, employment services, nursing, participant-directed supports, and family support services.
